| In each scenario, there are
ACHS Guidance forms (Transcript Request Form and the Application
Processing Checklist) which must be filed in Room 319. When the requested
information is mailed from Carroll, the "checklist" will be
returned to each student in homeroom. Students should come to the
Guidance Office if they have not received a "checklist" after 10
days.
Stop by the Guidance Office in order to have an official high school transcript sent out to the college/organization. You may pick up copies of these forms in the Guidance Office or online from www.jcarroll.org. There is a $3.00 charge for each transcript. Students who elect to have the Guidance Office mail the applications must provide proper postage and an envelope. |

Note: There is a December 7th deadline in order to guarantee pre-Christmas college application processing. |
