Parent Informational Letter    
AJCHS will be partnering with Maxim Health Systems to offer a one day on-site FLU vaccination with the FluMist Vaccine, an intranasal flu vaccine.  This program is optional and is being offered for your convenience. Please look for the informational letter and consent form in the mail or you can use these on line links.
Consent Form    -  Please submit completed form prior to 10/04/10
FORM SHOULD BE RETURNED TO SCHOOL NURSE ALONG WITH PAYMENT
